Three discounts are set to end on December 31, 2026: the $199 Wegovy shot offer for new patients, the reduced Foundayo prices on two doses, and the current Zepbound savings card. On January 1, 2027, Wegovy's list price drops to $675, Medicare's negotiated price for semaglutide starts, and the Medicare GLP-1 Bridge carries on at $50. New 2027 self-pay prices haven't been announced yet. We'll update this page as soon as they are.

Other prices have no announced end date, including the Wegovy pill ($149 to $299), the Wegovy prepaid plans ($249 to $329), Zepbound at LillyDirect ($299 to $449) and the Wegovy savings card. The makers can still change them at any time.
What's confirmed for January 1, 2027
- Wegovy's list price drops to $675 a month. That's about half today's list price, and the same price applies to Ozempic and Rybelsus. It matters if your insurance makes you pay a percentage of the list price (coinsurance) or the full price until your deductible is met. Novo Nordisk says it doesn't change self-pay prices.
- Medicare's negotiated price for semaglutide starts. Medicare will pay $274 for a 30-day supply of Ozempic, Rybelsus or Wegovy, down from about $959. This isn't your copay: your Part D plan still sets what you pay. Part D covers Wegovy only for approved uses such as lowering heart risk, not for weight loss alone.
- Part D limits go up. The maximum deductible rises to $700 (from $615) and the yearly out-of-pocket cap to $2,400 (from $2,100).
- The Medicare GLP-1 Bridge continues. Eligible Part D members keep paying $50 a month for Wegovy, Zepbound KwikPen or Foundayo through December 31, 2027. See who qualifies.
- The BALANCE program won't start in Part D in 2027. Medicare had planned a second GLP-1 program for Part D plans in January 2027. It was put on hold in April 2026 because too few plans signed up.
- Most deductibles reset. If your plan runs on a calendar year, you start paying toward a new deductible in January.
What we don't know yet
- The Wegovy shot price for new patients after December 31.
- 2027 prices for Foundayo 2.5 mg and 5.5 mg.
- The terms of a 2027 Zepbound savings card.
- Any change to Zepbound's list price. We found no announcement from Lilly as of October 10, 2026.
- Whether your employer keeps covering weight-loss medicines. In a 2026 survey of large employers by the Business Group on Health, 67% covered them, and 72% of those expected to keep doing so in 2027. Mercer found 5% of employers with 500+ workers plan to drop or are considering dropping coverage for 2027.
What to do before December 31
If you pay cash
- If you and your prescriber have already decided you'll start the Wegovy shot, the $199 price covers your first two fills only if you start in 2026. Don't rush a medical decision for a discount.
- If you'll stay on Wegovy all year, compare the prepaid plans: $249 a month over 12 months instead of $349. Is prepaying worth it?
If you have insurance through work
- During open enrollment, check your plan's 2027 summary of benefits and drug list. Search for "weight management" and the brand names.
- Look for prior authorization, step therapy and quantity limits, and ask whether current users keep their coverage.
- If your plan uses CVS Caremark: since October 1, 2026, Zepbound can be covered again as a preferred option, but only if your employer chooses to cover it.
- If you've already met your 2026 deductible, ask your plan whether it allows a 3-month fill before December 31.
- If you use a savings card, check for the 2027 version in January. People had to sign up again when the 2025 card ended. How the cards work.
If you have Medicare
- Open enrollment runs October 15 to December 7. To use the Bridge, you need Part D drug coverage, through a stand-alone plan or a Medicare Advantage plan with drug coverage.
- If you take Wegovy or Ozempic for an approved use other than weight loss, check your plan's 2027 copay in your Annual Notice of Change or in Medicare Plan Finder.
